Quistes sinoviales lumbares

Son una causa infrecuente de dolor radicular y dolor lumbar

Los más frecuentes se encuentran a nivel L4-L5 y suelen estar asociados a una artropatía degenerativa de las facetas.

Aunque se han tratado de forma satisfactoria con la aspiración percutánea o con la inyección de corticosteroides, la extirpación quirúrgica es el tratamiento definitivo, sobre todo para quistes que causan dolor que no remite o déficits neurológicos.

Generalmente, una laminectomía o laminectomía parcial y facetectomía medial bastan para exponer el quiste, que puede estar fuertemente adherido a la dura.

Las complicaciones quirúrgicas incluyen lesiones radiculares,fístula de líquido cefalorraquídeo, hematomas epidurales, y la inestabilidad tardía, haciendo necesario la fusión espinal.
